Sydney Local Health District (SLHD) is seeking an experienced Clinical Nurse Consultant to join our Older People’s Mental Health Team at Canterbury Community Health.

We are looking for a registered nurse with extensive experience in providing mental health care in the community to people aged over 65 years.

The “Community Older People’s Mental Health Team” (OPMH Team) is a specialty service of the community mental health system. 
OPMH Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination.

Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.

Consumers under the care of OPMH Teams will either have:
1. Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months. 
2. A period of treatment as recommended by evidence based practices.

To be considered for this role you must have…

  • The ability to participate in a Monday-Friday roster
  • Current NSW Driver’s Licence (P2 Licence acceptable)
