Provide contemporary evidence-based consumer/person focused care in collaboration with the multidisciplinary care team to facilitate optimal health outcomes for our consumers. The role undertakes delegated activities while assuming individual accountability for actions within the position and context of the Rheumatology Department. The Clinical Nurse – Rheumatology is responsible for the clinical coordination of complex patient care in the Rheumatology Department. The specific patient group includes those patients with inflammatory arthritis who require complex drug therapies to optimise their condition. The role is accountable for case management, care coordination and patient education for patients who have complex rheumatic disease, and their families.

About the Role

  • Apply advanced clinical nursing/midwifery expertise, including assessment, planning, implementation and evaluation, to deliver a high standard of consumer/person centred care

  • Respond appropriately to, and provide support and direction to manage, sudden alterations in the health status of the consumer/person using advanced nursing/midwifery assessment and intervention skills.

  • Provide teaching to both consumers/persons and their families to facilitate learning and enable them to move towards independence.

  • Support an evidence-based learning environment and participate in learning activities/opportunities for self and others.

  • Participate in, and manage the performance and development planning of self and others

  • Integrate organisational policies and guidelines with professional standards by maintaining current knowledge and participating in relevant policy review.
