The Clinical Nurse – Community Health is a clinical nurse who contributes to the provision of high-level community nursing support through care coordination, planning and support for clients with multiple co-morbidities and their carers across the continuum of care. The Clinical Nurse practices autonomously and within a multidisciplinary team to provide nursing services within a rural health service in a community setting.
