etro South@Home are looking for two Senior Health Practitioners (Occupational Therapist / Physiotherapist / Social Worker) to join their Aged Care Assessment team (ACAT). ACAT is a multi-disciplinary team of allied health and nursing clinicians whose main function is to comprehensively assess the care needs of frail older people and assist them to gain access to the most appropriate types of care, including approval for Commonwealth subsidised aged care services.

About the Role

The purpose of this role is to deliver senior Occupational Therapy / Physiotherapy / Social Work services within the Aged Care Assessment Team. In delivery of assessment services, the role operates within the Aged Care Act 1997 and its Principles and aligns with the Commonwealth Department of Health guidelines and practice recommendations.

This includes:

  • Delivering high quality evidenced based Occupational Therapy / Physiotherapy / Social Work clinical services to patients/consumers within a highly functioning multidisciplinary team.
  • Contribution to the advancement of quality Occupational Therapy / Physiotherapy / Social Work services and optimal patient care, through provision of service development, quality improvement, and clinical education.
  • Monitoring and reporting on work practices and outcomes, including initiating, planning and evaluating relevant quality and research initiatives.
  • Providing supervision and training to less experienced (HP3) clinicians and students
