  • The opportunity

    We are currently seeking suitably experienced Nursing and/or Allied Health professionals to join our highly dedicated team as a Health Navigator. This is an exciting time to come on board with Silver Chain as they deliver the new Health Navigator services in partnership with the Government. This role will see you out and about in one of our Silver Chain cars working in both the hospital and community setting assisting people to “navigate’ the health system.

    This service is aimed at reducing unplanned hospital admissions and as the Health Navigator you will work with clients suffering with chronic and complex health conditions. Following careful assessment of the client, you will identify gaps in their care and support and provide a holistic service by partnering with GP’s and other healthcare professionals as part of the integrated care system.     Should you be successful you will be lucky enough to embark on our comprehensive 3-week onboarding and induction program and on completion be awarded a certificate in Chronic Disease Management.

    We are proud at Silver Chain to have specific IT programs created to support you in your role as a Health navigator. We have developed the program so clients can easily access it via phone and video conferencing in the safety of their own home anytime. 

    On offer are permanent full-time positions based out of the Sunshine office. You are required to be available to work across a rotating 7-day roster with shifts between 7am and 10pm.

    About you

    The ideal candidate is committed to supporting clients and understands the importance and value of individualised, holistic health care. You will be able to meet the following criteria:

  • Relevant qualifications in Nursing and/or Allied Health
  • AHPRA Registered
  • Demonstrated Acute / Clinical experience
  • Minimum 4 years post-graduate clinical assessment and technical skills
  • Well-developed communication skills (written, verbal and listening)
  • Critical thinking and problem-solving abilities to make informed decisions about client care needs
  • Ability to work cooperatively across interagency and multidisciplinary teams
  • Adaptability to operate new technologies and emerging information
  • Confident developing care plans in partnership with the client and other health services professionals
  • Experience providing care for complex health care needs in the community
  • Emergency Department experience would be advantageous
  • Under the Aged Care Act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
