Health Systems Navigator
We are currently seeking suitably experienced Nursing and/or Allied Health professionals to join our highly dedicated team as a Health Navigator. This is an exciting time to come on board with Silver Chain as they deliver the new Health Navigator services in partnership with the Government. This role will see you out and about in one of our Silver Chain cars working in both the hospital and community setting assisting people to “navigate’ the health system.
This service is aimed at reducing unplanned hospital admissions and as the Health Navigator you will work with clients suffering with chronic and complex health conditions. Following careful assessment of the client, you will identify gaps in their care and support and provide a holistic service by partnering with GP’s and other healthcare professionals as part of the integrated care system. Should you be successful you will be lucky enough to embark on our comprehensive 3-week onboarding and induction program and on completion be awarded a certificate in Chronic Disease Management.
We are proud at Silver Chain to have specific IT programs created to support you in your role as a Health navigator. We have developed the program so clients can easily access it via phone and video conferencing in the safety of their own home anytime.
On offer are permanent full-time positions based out of the Sunshine office. You are required to be available to work across a rotating 7-day roster with shifts between 7am and 10pm.

Under the Aged Care Act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
