Division of Medicine are looking for a Nurse Practitioner – Navigator to join their Chronic Disease Nursing team. The Chronic Disease unit aims to deliver evidence based care to reduce chronic disease burden across the health care continuum, through empowering patients with contemporary education, medication management and primary care engagement. This is done in collaboration with cardiology infection diseases specialists and General practitioners.

About the Role

The role of the Rheumatic Heart Disease and Complex Cardiology Nurse Practitioner/Navigator will:

  • Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required.
  • Provide safe, quality, cost effective care and practice within the Nurse Practitioner’s approved Practice Scope that is consistent with contemporary standards and evidence-based practice.
  • Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs.
