About your opportunity ( )

Mount Isa Community and Primary Health Care provide expert support, advice and collaborative planning to ten facilities that comprise the North West Hospital and Health Service region. It promotes the Enhanced Model of Primary Health Care as the preferred framework for delivery of rural health services, through training, education, strategic planning and resource allocation.

The Nurse Navigator Service is a state-wide model of care designed to provide support to patients with complex health conditions requiring a high degree of clinical care. This integrated and proactive approach works across system boundaries, including close partnership with patients and families, other health and allied-health services ensuring patients receive timely and appropriate care. 

This position forms part of this state-wide model of care and is responsible for providing advanced professional and clinical leadership in the coordination and delivery of comprehensive and complex care, education and consultancy and portfolio management to ensure safe practice and high quality outcomes.

What we are seeking:

  • Proven advanced clinical knowledge, skills expertise and leadership in the delivery and coordination of patient focused nursing care within a Nurse Navigator / Nurse Practitioner scope of practice and model of care.
  • High degree of systems and models of care literacy and can manage care across a variety of health systems to maximise outcomes.
  • Proven ability to engage in complex decision making and critical thinking, integrate information and/or evidence, judiciously use clinical investigations and skilfully and empathetically communicate with all involved in the care episode.
  • Requirements:

  • Proof of Master of Nursing – Nurse Practitioner qualification, registration with AHPRA and a current practising certificate are mandatory requirements.
  • If you do not possess the neccessary requirements for the Nurse Grade 8 level you can apply as a CNC / Nurse Practitioner Candidate ( ) @ the Nurse Grade 7 level.
  • The position may involve travel to various facilities or units within the North West HHS.
  • Class C motor vehicle licence.
  • This is a Vaccine Preventable Diseases (VPD) risk role.

    What’s on offer?

    Permanent full-time role, 76 hours per fortnight.

    Generous remuneration package value up to $156,161 p.a. comprising:

  • salary range between $130,713 to $136,465 p.a. (Nurse Grade 7)
  • annual recreational leave p.a. with 17.5% loading
  • 12.75% employer contribution to superannuation.
