What you’ll be doing

  • Acting as the central point of contact for clients
  • Implementing, monitoring and modifying in-home care plans to meet client needs
  • Supporting clients to achieve their personal and clinical goals within their home environment
  • Responding to enquiries from clients, referrers, service providers and funders
  • Supporting the medical administrative team to allocate service providers and complete records and documents
  • Helping to improve the health and wellbeing of hundreds of Australians.

  • Your skills and experience

  • Qualification in occupational therapy or physiotherapy with current AHPRA registration
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
  • Excellent clinical reasoning and problem-solving skills
  • Excellent verbal and written communications
  • Strong administration and time management skills, tech savvy
  • Passionate about health and committed to making a real difference in the community.
