The Opportunity

We are currently seeking an experienced full time Support Coordinator who will be responsible for assisting in the choice of services and supports, as well as coordinating the provision of and monitoring the delivery of services and support across our Accommodation & Care Solutions business.

As Support Coordinator you will build and maintain positive, long-term and collaborative relationships that will ensure the delivery of reliable, quality, individually tailored, compliant customer care and solutions to our aged and disability customers, enabling them to optimise their independence, health, well-being and quality of life.

Essential

  • Certificate IV or higher in Community Services / disability work or Cert IV Diploma in Case management and/or have extensive demonstrated experience in a Case Management or Care Coordination or similar role
  • Knowledge of the National Disability Insurance Scheme (NDIS) and the disability sector
  • High professionalism and experience working collaboratively with multiple stakeholders in formal and informal partnerships to achieve outcomes
  • High standard and ability to articulate information, factual and evidence based written reports
  • A practical general knowledge of a human rights-based approach and the individual and community context, and sector and organisation purpose and values
  • Applies the rights-based approach and sector and organisation values in own work
  • Effective listening skills and seeks, provides and/or shares practical
  • information in an appropriate and respectful manner
  • Adheres to reporting, documentation and administrative requirements
  • Uses relevant communications and technology systems
