As part of the Behavioural Supports Ontario Program and GAIN team, the clinician will provide direct support and expertise to clients living in their homes who are experiencing responsive behaviours related to dementia, mental health, addictions or neurological disorders. The role involves comprehensive bio‑psychosocial assessment and intervention, liaison with hospital, GAIN Community teams, GEM nurses, CCAC, and other practitioners, and leading person‑centred behavioural care planning, coaching, education, and behavioural modelling.
