Care Coordinator

Overview

Care Coordinators are the backbone of our practice. You will be responsible for ensuring every client has a seamless experience—from their first phone call through their entire treatment journey. You will manage scheduling, handle patient communication, coordinate care between providers, and ensure follow-ups happen on time. This role is ideal for someone who is highly organised, empathetic, and enjoys working in a fast-paced healthcare environment. You will be the voice of Nafs Wellcare for many of our clients, setting the tone for their experience with us. As we grow, this role offers significant opportunity for advancement.
