Job Title: Care Manager – Social Work (MUST HAVE LMSW or LCSW along with MSW)

Department: Care Delivery

Reports To: AVP of Care Delivery Operations

Responsible for overall care management and quality of care for participants. Reviews assessments, coordinates holistic care plans across all domains, and ensures culturally sensitive, person-centered care.

• Participate in interdisciplinary care planning meetings and represent social work discipline


• Review documentation for quality; address deficiencies with staff and follow disciplinary policy


• Monitor, coach, and conduct training sessions for field staff
