Position Overview
SITE - Rehab & Nursing Kentridge - 4118 Kalamazoo Ave SE - Grand Rapids
Schedule: Part Time, 8 hrs a week,
6:30 a.m. to 2:30 p.m. and 4 p.m. to 7:30 p.m.
Job Summary
Answers customer/care team phone calls to assist with the nutritional care of patients by initially screening patients for nutritional risk, coordinating menu selection, educating patients that are on less complex diets and maintains pertinent patient information in appropriate software program to ensure the patient's nutritional needs are met.
Essential Functions
+ Assists with meal selection utilizing appropriate menu program while stressing appropriate restrictions and good nutrition.
+ Screens patients regarding nutrition risk, obtains diet history, allergies, food preferences.
+ Accurately maintains pertinent patient information.
