Communicating with Persons Outside the Organization:
Dementia Navigators connect caregivers and individuals living with dementia to community resources and clinical support, helping them remain safely at home as long as possible. Based in Henry County, this role serves Henry, Benton, and Decatur Counties, with some travel across Tennessee.
Key Responsibilities
Communicating with Persons Outside the Organization:
