Job Overview


This is an exciting opportunity for a pharmacist to take a leading role in the management of long-term conditions, helping to improve patient outcomes while working closely with GPs and the wider practice team. The successful applicant will lead and support the management of patients with long-term conditions including:

  • Asthma and COPD

  • Type 2 Diabetes

  • Hypertension

  • Chronic Kidney Disease (CKD)

  • Cardiovascular disease prevention and lipid management
