Home Health RN Case Manager

We are seeking a dedicated Registered Nurse Case Manager to provide skilled, patient focused home health care within Luzerne County. This position is responsible for managing a defined territory and delivering comprehensive nursing services across the home health continuum. The role is 100 percent patient care and requires collaboration with team members to ensure continuity of services when covering for scheduled time off.

• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• Weekly Hour Guarantee: 32 hours


• Productivity Expectation: 30 points per week


• Coverage Area: Luzerne County


• On Call/Weekend Requirement: Every third weekend and one night of on call coverage per week
