Position Overview
Intake Specialist Job Location: Wallingford Center, Connecticut
Full Time - Day Shift
9:00am - 5:30pm
*This role will be a combination of on-site in Wallingford, CT and remote*
Accepts and processes referrals, completes full data entry of referral, obtains and verifies insurance information and requests initial authorization for Home Health and Hospice admissions.
Essential Duties and Responsibilities:
+ Accepts and processes appropriate referrals to the agency per agency policy in collaboration with Intake Nurses and coordinates all intake information in the management of referrals for service.
+ Assures coordination of all required services to clients and families by agency staff and community resources.
