Helps patients develop or regain skills needed for daily living and independence after illness, injury, or disability. Designs and implements individualized therapy plans to improve quality of life.Key Responsibilities1. Evaluate physical, cognitive, and functional limitations.2. Develop and implement individualized therapy plans.3. Instruct patients and caregivers on adaptive strategies and equipment use.4. Track progress and adjust interventions as needed.5. Collaborate with physicians, PTs, SLPs, and nursing staff.6. Document treatments and outcomes in EMR systems.7. Contribute to discharge planning and patient education.
