Salary Compensation:

$290,000 - $320,000 salary; approximate comp. after productivity bonus is $314,000 (15 visits per day) - $464,000 (20 visits per day); Comp includes base salary + Sign bonus + Quarterly bonus + Retention bonus + Patient visits productivity bi-weekly bonuses + 401K + Full robust benefits package. Relocation bonus if required.
