Position Overview
DUTIES:
Uses nephrology nursing standards of practice to prepare for patient clinic visit, complete patient assessment during visit and provide appropriate follow-up care, post visit, in collaboration with multidisciplinary team.
Provides, as member of multidisciplinary team, group/individual patient/family education related to all stages of renal disease to optimize renal function.
Supports Modality Coordinator in providing Kidney Care Clinic patients and family addition education regarding treatment options
Provides quality education, monitoring, telephone and in-home support, as indicated, for independent dialysis (PD) patients. Collaborates with external service providers regarding PD services, supply deliveries and inventory.
Performs peritoneal exchanges to inpatients receiving peritoneal dialysis.
