Job Summary

The SIL Outreach Support Worker meets individuals either at their home or in the community and provides emotional and practical support to foster independence. The support to the person is individualized and is required to address challenges, access resources, develop life and social skills and help build a support network for the individual. Community access with the individual is required. This position does require a high degree of collaboration with the SIL Outreach Workers who will maintain administrative oversight to the individuals in the SIL program. Some administrative duties like completing contact notes, mileage claims and filling out incident reports as needed are also required.

Responsibilities

  • Monitor and report progress of the individuals
