What will your responsibilities be?
Undertake Strengths-Based Needs Assessments and review care packages for adults with severe and enduring mental health needsUpdate care plans and complete Mental Capacity Assessments where requiredCarry out safeguarding work in line with statutory duties when appropriateSupport timely hospital discharges for individuals under secondary mental health servicesWork with both in-borough and out-of-borough placements; understanding of mental health pathways is beneficial Benefits:
