Speech Therapist, is responsible for providing rehabilitation services for home based clients in need of speech therapy to assist patients in communicating and swallowing appropriately, including assessing patients, diagnosing disorders that can impact swallowing and speaking and providing treatment for their patients
Primary Functions: Accepts responsibilities for all identified duties. Visits patients in home/hospital. Performs initial and ongoing assessments, including Oasis assessments at appropriate time points and evaluates the level of function and identifies problems. Develops initial and ongoing treatment plans, including goal setting Provides treatment in accordance with physician orders.
