Transition Aged Care Program – Home Care Worker – Lower North Shore

Silverchain is seeking experienced Home Care Workers to join our Transitional Aged Care Program (TACP). The TACP program supports older people transitioning from hospital back to independent living at home. This short-term, rehabilitation-focused program assists clients recovering from extended hospital stays and helps them achieve their recovery goals. We are looking for TACP Home care workers to join Silverchain team in the Lanecove area and Surrounds.

What’s on offer

• Part time Award rate $37.53 + Super + Km’s

• Monday to Friday roster - between hours of 7 am and 5pm (please note: must be available 2-3 days a week).
