Being part of the broader Public & Community department, this is a great opportunity to join the team in this full time, fixed term role until June 2025, based at our offices in Doveton. A Day in the Life In this role, Team Leader – Aged and Rehabilitation Stream, you'll not only be part of a supportive and high functioning cohort, but you'll also work collaboratively within the multi-disciplinary team to enhance the client's journey through Monash Health Community. Leading a team of 12, spanning Community Development, Allied Health, Nursing and Administration, you'll have responsibility to ensure referred clients are triaged, registered and booked for appointments with the most appropriate service for their needs. You'll support the Access Manager and team to ensure the delivery of Access performance is in line with the directorate objectives and targets. In addition, you'll develop and maintain relationships with key stakeholders and the local community, providing clinical services in line with the discipline scope of practice across the portfolio.
