As an experienced Dietitian with Empower, you will have the opportunity to work with a variety of patients across home care in the Northern and Southern suburbs of Perth. This role is a 12-Month Maternity Leave Cover. You will: Complete comprehensive nutritional assessments for residents and for clients in their own home through home care packages/NDIS.Develop personalized nutrition plans and goalsSupport the dietary requirements of individuals with difficulty swallowing and chewing, bowel and intestinal issues and other complex issues.Provide weight management, poor appetite and dysphagia interventionsManage food intolerances and allergiesProvide education on diabetes managementEducate clients and their families to optimise their ability to live healthy lives within the communityWork holistically with other allied health professionals to provide a multidisciplinary care planLiaise with service providers to ensure effective communicationEnsure caseload and documentation are timely and completed at a high standard You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.Do you have what it takes?
