We are a leading home care provider dedicated to enhancing the quality of life for older Tasmanians. Our team is passionate about providing personalised care and support to help our clients remain independent and active in their communities.As an exercise physiologist at Care Forward, you will use a wellness approach with clients who want to improve their functionality. This may be related to an acute or chronic condition, age, or after injury. You'll conduct intake assessments, develop custom exercise programs with your clients, and deliver services promoting recovery and rehabilitation.Your role will involve developing progressive exercise programs to improve strength, flexibility, endurance, or circulatory function. Inspire client confidence and motivate them to participate in individual activities and group programs while monitoring and documenting client progress.We are open to candidates based in Tasmania's south, north, or northwest and have office space in Hobart, Launceston, Devonport, and Burnie.Full-time, part-time, casual and contractor positions can be considered for the right candidate.A competitive salary packageThe opportunity to work with a leading home care providerA great work-life balance with flexible hoursThe chance to make a real difference in the lives of older TasmaniansOngoing training and development opportunities to enhance your skills and career progressionA supportive and collaborative team environment.You will doPrescribe exercise and support clients' behaviour change to help them manage their health independently.Create goal-oriented programs with clients to work towards their rehabilitation or recovery and document progress.Help participants improve their strength, flexibility, endurance, or circulatory functions.Demonstrate how to complete exercise routines and use the equipment correctly.Inspire client confidence and motivation to participate in activities.Liaise with referral agencies, service providers, consumers, families and support networks to ensure the best client outcomes.Participate in stakeholder engagement, referrals, and promotion of the company's programs.You need to haveCurrent registration with ESSA as an accredited exercise physiologistSelf-drive to continually learn and improve in your roleExcellent relationship, team building, and communication skillsThe ability to work autonomously and as a member of a multidisciplinary teamA commitment to a positive and inclusive workplace cultureGood problem-solving and decision-making abilities, even in high-pressure situationsA current driver's licence and access to a reliable vehicleAn up-to-day first aid and CPR certificate.Ideally, you also haveDemonstrable experience in providing evidence-based clinical assessment and planning appropriate interventions for clients with chronic or complex conditionsComprehensive understanding of community-based service provision
