Role Overview:
Join us in an exciting journey as a Documentation Registered Nurse! Dive into a role where you organise care, crafting plans to meet residents' needs. Assess, plan, implement, and evaluate care, igniting positive change. Your actions will shape lives – ready to make a difference?
Key Responsibilities:
- Collaborate with the clinical leadership team for accurate assessment and documentation.
- Mentor the team for person-centred care practices.
- Assist with AN-ACC and care planning.
- Complete precise clinical documentation using Leecare assessments.
- Develop resident care plans based on assessments.
- Review care plans accordingly.
- Ensure all clinical assessments are fully documented.
- Identify residents' health needs through assessments.
- Provide quality residential care according to standards.
- Seek opportunities for service improvement.
- Offer holistic care in consultation with stakeholders.
Skills & Experience:
- Bachelor’s Degree in Nursing.
- Registration with AHPRA.
- Current DHS NDIS Worker Screening Check and National Police Clearance.
- Minimum 2 years post-registration experience in Aged Care.
- Experience in admission processes.
- Proficiency in documentation, assessments, and Care Plans review.
