Community Registered Psychiatric Nurse - Mental Health Clinician Community Key Clinician – Registered Psychiatric Nurse RPN3Adult Community Mental HealthTeam with homeless portfolio based in CBD of MelbourneCombination of assertive outreach in the community and clinic-based workCommunity Team 1 (CT1) provide care to consumers living in the CBD and the Inner West of Melbourne. Additionally, they hold the homeless portfolio for Royal Melbourne Hospital Mental Health Service (RMH MHS). CT1 applies an Assertive Community Treatment (ACT) model of care, which is characterized by lower caseloads, community-based treatment (outreach) a team-based approach and high intensity of care. This can involve medication management, psychosocial support, assessment and referral (housing, financial, vocation, increasing levels of participation), psychological assessment, formulation and interventions, crisis assessments and admissions.
We are seeking a Registered Psychiatric Nurse Grade 3 (RPN3) or a suitably experienced RPN2 ready for the challenge to work in the community as an RPN3.
This is a unique opportunity to work with complex consumers in a supportive team environment and to make a difference for some of the more vulnerable consumers in our community.
